Plantar wart, keratosis, or something else?

39M, 6'2" Caucasian 200lbs. I've had this painful growth on my foot at the pressure point behind my little toe for the better part of my 30s. I've seen several primary care doctors and two podiatrists and still am not 100% certain what this growth actually is due to conflicting diagnoses.

The pain feels like stepping on a nail when it's left untreated and I apply full pressure to it. After it's been treated (i.e. shaved down with a scalpel), the pain is basically gone until it grows back again.

The first podiatrist diagnosed it as a keratosis (iirc) and also mentioned an alternative name "horny lesion". This doctor was probably close to 70 years old and was on the verge of retirement. He treated it by scraping it down and removing all of the hard flesh, which was painless until he dug all the way down and blood was drawn. He said it would be there permanently unless I had an unadvised surgical procedure which would put me on crutches for weeks and may not ever heal to my satisfaction.

Since that doctor retired, I was referred to another doctor and she diagnosed it as a plantar wart. Each time I returned, she seemed perplexed that the 'wart' had grown back. She would shave it down until it bled, apply a prescription strength salicylic acid, wrap it in a bandage and tell me to not get it wet for 24 hours. After that, treat at home with an OTC salicylic acid. I told her of the differing diagnosis from my previous podiatrist several times, and she stayed firm about it being a wart.

I saw her around a half-dozen times until my insurance changed and it started costing me $260 to have this tiny thing treated. On my last visit, I made note of the type of scalpel they used, watched their technique and then watched some youtube videos of how they're removed in the doctor's office. I've been doing it myself for probably close to two years with no ill effects.

I turn to Reddit to hopefully get a more affirmative diagnosis. Is there any way I can get rid of this thing for good without a more involved surgical procedure?

I only thought to post on Reddit after beginning the excision process today. The first picture is from 1.5 years ago, but it's the closest thing I have to a 'before' picture. This is the 'growing back' phase. Over time, it will eventually smooth over and almost not be noticeable at first glance.

I try to not go very deep because I'm an amateur and I know I'm taking a bit of a risk by doing this myself. Rest assured that I sterilize everything thoroughly beforehand. Thankfully, it feels much much better after I do the procedure myself, and I've never had any complications.
