Adoption Records

I am wondering how I might go about finding adoption records for my great grandfather. I suspect he may have been placed for adoption at an early age after his mother died (or was institutionalized?) Then reverted back to his father's surname as an adult. It's like he didn't exist until he was in his 20s and getting married. He was born we believe in obion TN in 1911.
