Nail plate seperating from wood truss in ceiling.

Hello, I’m just wondering if anyone can give a reason as to why the nail plates on the wood truss in the roof would caused the wood to come out. We have ceiling sagging and water ingress in parts of the roof/ceiling. It seems to be coming undone in spots of the ridge capping.
