Is someone misusing mParivahan app?

Can someone please help? How worried should I be on a scale of 1 to 10?

I am from Mumbai.

Last evening I received 4 SMSes in quick succession...

the first two mentioned a Verification Code (OTP) for availing Vahan Citizen Service... But the thing is that I did not request for any such service so why did I receive the OTP.

the next SMS was that transaction of Rs 250 was successful - I did not do any such transaction, and neither does it show on my bank account

the last SMS was "Please submit the print copy of the application form, along with original RC and other original documents to concerned Dy / Regional Transport Office.

I can't seem to be able to attach screenshots in this post.... else I would have.
