Site/app that explains jargon speak on pathology reports

Awhile back, someone posted a link to a site or app where you can copy and paste the medical jargon on a pathology report and the site or app explains what it means in layman terms. It then lists five questions for you to consider asking your doctor. I’ve searched the last posts to try to find this site or app, but I’m coming up empty. Does anyone know of the site or app that I’m describing? Or am I way out to lunch?
