Failing skill checks that are good or great

So recently I have noticed I’ve been blowing up gens even when getting good or even great skill checks. I screen recorded and go back to re watch the clip and i definitely get them right, but still end up blowing the gen. it seems to be more frequent now, I blew up the gen 3 times in a row on my last game and ended up getting killed because of it. It’s making the game unplayable now as I’m the first one to get into a chance most games and then again as soon as I get on a gen. sometimes I blow the gen without even getting skill checks too, the gen doesn’t actually blow for me, but the bar goes down and it pauses so I know when it happens. Has this happened to anyone else before? Is there a fix to it?
