Why is the judgement so confusing?

The judgement mentions residence based reservation as unlawful and not the state quota itself.However it mentions that these state quota seats apart from some 'Institutional reservation' should be filled ny merit through NEET PG.Does institutional reservation mean where one did their MBBS from?This is so confusing .Can someone please clarify about this?
